Tap or click on the registration task on your home page banner or select "Settings" at the top right hand side of your screen, and select "Add Child/Dependent" from the drop down menu. The Patient Portal can’t replace in-office visits, but it does permit families to perform many administrative tasks without calling the office and saves time! Can you preregister your child's demographics?

Demographics: Patients can pre-register their child’s demographics via the Patient Portal but the Patient Information Form needs to be completed. Any future changes can be made directly on the Patient Portal. For your convenience, the Patient Information Form on line is fillable and can be printed.
