For example, your visit notes, scanned documents and forms, and records we receive from other providers will not be available on the Portal.

Outside Requestors

Providers, STAT and urgent request requests for records should be directly faxed to the appropriate clinic for processing. Sending these requests to Healthmark will cause the request to be delayed. If you are sending records to us, these should also be sent by fax to the appropriate clinic —not to Healthmark.
