How do I access information from the previous patient portal?

Previous Portal Users will need to create an account with MyHealthHome in order to continue accessing their personal health information. Information contained in the previous patient portal can still be accessed by contacting your local hospital.

How do I use LMC MyChart?

Visit LMC MyChart on your computer. Download from the App Store or Google Play . Custom shortcuts and a searchable menu makes it easy to find what you’re looking for. Make an appointment, get reminders, and complete eCheck-In. You can also search for nearby urgent care or emergency departments to let them know you’re on the way!

How do I keep my medical information secure in MyChart?

As a MyChart user, your role in maintaining the security of your medical information is: 1) Changing your password on a regular basis, and 2) Keeping your login ID and password confidential. Your activities in MyChart may be tracked by computer audit trails and the entries that you make may become part of your electronic health record.

What to do if you need medical emergency?

If you are experiencing a medical emergency or need urgent medical care, immediately call 911, go to your local emergency room, and/or call your physician. MyChart is not your complete medical record. MyChart provides you with access to some (but not all) of the information contained in your electronic medical record.

Does Lexington Medical Center respond to messages?

Lexington Medical Center will respond to your message in as timely a manner as is reasonably possible. It is your sole responsibility to monitor your account for messages and other information and to respond and /or act (or not) upon them in a timely and appropriate manner. IV.

If you do not wish to be bound by these terms and conditions, do not access or use MyChart. The use of MyChart is completely voluntary and is not required to receive health care from Lexington Medical Center. Individuals must be age 16 or older to request a personal MyChart Account.

Is Lexington Medical Center copyrighted?

Lexington Medical Center allows you to view or download a single copy of the material on MyChart solely for your personal, noncommercial use. The compilation of all content on MyChart is the exclusive property of Lexington Medical Center and is protected by United States and international copyright laws.
