PATIENT ACCESS

Patients can access their online records and fill out necessary forms through our Electronic Medical Assistant (EMA). To gain access to the EMA Patient area, you must call our office and register as a new patient with our staff. We will then provide you a user name and password to enter the patient area.

VISIT PREPARATION

All new patients must complete the New Patient Paperwork prior to exam time. Click on the link below, then print and complete the forms. Remember to bring all paperwork with you to your scheduled appointment, along with a photo ID and your insurance card. We look forward to meeting you!

When possible, review your portal instructions before 3:00 pm so that you may speak with a nurse, while we are still in the office, if you have questions about your instructions. Questions regarding information viewed on the Portal should be directed to your team during regular office hours (Monday-Friday 8:30 am-4:30 pm).
