If the proxy’s legal relationship to the patient changes, the proxy must notify Lexington Medical Center immediately. Proxy access to the MyChart Account of an adult patient over the age of 16 who is subject to a guardianship or healthcare power of attorney will automatically terminate one year after establishing proxy access.
Individuals must be age 16 or older to request a personal MyChart Account. Parents or guardians of minor patients, patient representatives, or other individuals may request access to a patient’s MyChart Account by following the terms outlined in the "Proxy Access" section of this Terms and Conditions statement. If you would like to sign up and don’t have an After Visit summary from your physician or a Baptist Health facility, visit MyChart to get an activation code. Once you have your activation code, you can setup your username and password on the MyChart website .
To login to MyChart, visit mychart.baptisthealth.com. Once on the page, enter your username and password that you setup when your created your account and click Sign In.
After logging into your MyChart account, hover over the Messaging tab and select “Ask a Question”
