For access to the LMC MyChart Account of an adult patient or a minor patient ages 16-17, the patient and the proposed proxy complete the “Proxy for Adult Application.” Proxy access to an adult patient’s MyChart Account will automatically terminate three years after establishing proxy access.

Certain portions of MyChart permit you to securely exchange electronic messages and other information with Lexington Medical Center providers. You agree that you will not use MyChart, including its messaging functionality, for emergency or other urgent healthcare needs, or as a substitute for actual medical advice and care by a physician. You also agree that you will use the messaging function appropriately, and not send excessive, unnecessary, or inappropriate messages through MyChart.
Lexington Medical Center reserves the right, at its sole discretion , to discontinue your MyChart Account and use of MyChart. The following is a list of reasons which may result in deactivation of a user’s MyChart Account. The list is not exclusive and each circumstance will be evaluated individually.
Lexington Medical Center will respond to your message in as timely a manner as is reasonably possible. It is your sole responsibility to monitor your account for messages and other information and to respond and /or act (or not) upon them in a timely and appropriate manner. IV.
