The Patient Portal provides you with secure, online access to portions of your medical records, as well as an easy and convenient way to communicate with the practice through a secure internet connection. Think of it as an online account for your doctor’s office, similar to online banking or credit card accounts you may currently have.

Please note the patient portal is not monitored 24 hours a day, 7 days a week; we will respond as soon as possible but expect a minimum of 2-3 business days . New patients are asked to arrive 30 minutes early to complete paperwork. You can download, print, and complete the forms prior to your appointment. Please bring the completed forms, a list of all medications you are currently taking, recent copies of x-rays, lab results, photo ID and your insurance card to your appointment.
Please contact your pharmacy for refill requests. Your pharmacy will fax us the request and your refills will be processed within 72 hours. You may also request refills on the patient portal.
We require payment of your co-payment and past-due account balances at the time of service. We accept cash, checks, debit and all major credit cards. (Visa, Master Card, American Express, Discover.)
We will submit claims to most major insurance carriers. Please bring your insurance card and I.D. with you to every visit so we can ensure that our records are accurate.
To better serve the needs of all our patients, we ask that you call at least 24 hours in advance to cancel or reschedule appointments. We make every effort to stay on schedule, although emergencies arise.
