What is the patient portal and how does it work?

The Patient Portal provides you with secure, online access to portions of your medical records, as well as an easy and convenient way to communicate with the practice through a secure internet connection. Think of it as an online account for your doctor’s office, similar to online banking or credit card accounts you may currently have.

How do I contact MyChart or myjeffersonhealth?

You can use the Guest Estimates option as well! URGENT MEDICAL MATTERS: Please do not use MyJeffersonHealth to send messages requiring urgent attention. What is Patient Portal?

The Patient Portal can allow you to access discrete pieces of your personal care data including: Any medications prescribed by your provider. A record of your appointments, including your upcoming visit.

What is Jefferson Comprehensive Health Center?

It is a secure, online personal health record that offers advantages for both you and your provider to make your visit go faster and smoother. Having this online portal also gives you, the patient, access to your health record with information readily available at your fingertips. That’s why we are asking our patients to SIGN UP TO USE OUR PATIENT PORTAL.

Where is the portal in JCHC?

JCHC has a portal in the Main Lobby.

What is My Chart?

My Chart Providence Washington offers secure online access to portions of your electronic health record (EHR), called Epic. It enables you to securely use the Internet to receive and help manage information about your health, such as prescription refills and health records.

What is OpenNotes

Each time you have an appointment or discussion with your clinician, he or she describes what happened in a note. Notes often start by listing the symptoms or questions you have.

Step 1

  • An email with “Portal login information from your doctor’s office” has been sent to you. When you open that email, you will find your Login Credentials with your username. Within that section of the email, there will be an orange button “Set Up Portal Account.”

  • After clicking the orange button, the user validation process will begin. You will be asked to select a phone number and a verification code will be sent to that number. You can select to receive the verification via voice or text. After making your selections, click “Send Code.” Your verification code will be sent and another screen will open asking you to enter the code you received. The ve…

Step 3

  • On the next screen, you will create your password. Click “Password Guidelines” to help create your password. Another password tip is to begin your password with an uppercase or lowercase letter.

Step 4

  • The next two pages will display consents to read—eClinicalworks consent and Practice consent forms. If you agree and wish to proceed with the portal setup, you will need to click the check box in front of “I have read the consent form and the above information” and then click “Agree.” The next screen is the Welcome page and you have successfully set up your portal account!
