How do I access the patient portal?

You must first obtain your patient code from the Medical Records department. During regular business hours, please stop by the Medical Records department or call the department at 636-2635. Once you receive your 16-digit code you can go to the Patient Portal.

The tragic death of Mr. T. S. Hawkins’ grand-daughter Hazel-his “Little Sunshine” - led him to make a public commitment that no other member of this community would have to die for lack of adequate health facilities.
