How to enroll in Healthmark?

To enroll, bring your personal (non-work) email address and photo ID to Healthmark Regional Medical Center Medical Records Department or Fax this form to 850 -951-4581. We will then email you the link and temporary password when your account is ready to access. When you log in, simply agree to the Patient Consent and create your own new password. This will ensure that your records are secure and confidential.
