Beginning in early December, your medical records for Grace Clinic® and Covenant Health Grace Surgical hospital will be accessible in our new patient portal, MyChart, offering you more convenience and giving your entire care team a more complete picture of your health.

In an emergency, call 911 or visit your nearest emergency room. Each member of your family must have their own individual Patient Portal account, as all messages and information transmitted become part of your permanent health record. Create a password that includes a mix of numbers, symbols, and mixed case letters.

If you believe your patient portal account has been accessed by someone else, call us immediately. We will disable your account and, if you would like to continue using the patient portal, we’ll issue you a new username and password at your next clinic visit.

You must have access code from your doctor. Each new or established patient is asked to fill out a Patient Care Form prior to seeing a physician. This document will allow your doctor to know your complete medical history to best serve your health care needs .

Can I request a refill through the Grace health portal?

Refill requests are currently not available through the portal. Thank you for being patient while we upgrade our system. If you need a medication refill please contact Grace Health directly at 269-965-8866 and follow the prompts for medication refills.

Accessing Your Account

  • If you forget your password, select the “forgot password” link on the login page. Once you’ve answered a few questions, you’ll be able to create a new password and access your account. After 8 failed login attempts, your account will be automatically locked. If your account is locked, you’ll need to bring an ID to your GraceMedclinic to verify your account.
