Your personal and medical information in the Health Record Home portal will be kept confidential. Health Record Home uses a secure, encrypted connection that meets the highest industry standards. All health data is stored on a secure server and managed with the protection of your information in mind.

How Will Our Office Utilize The Portal?

How Do I Create A Portal Account?

First, make sure our office has an accurate email address for you and we will send you an email with an “invitation link” to create your portal account.

Provide your email address during checkin or checkout at your next visit.

Receive Welcome Email

You will receive a welcome email with instructions to follow. This will walk you through setting up your portal account.

Sign In

Sign in with your credentials that you previously setup or with one of the Alternative methods.

How to request a MyHealthRecord account?

You can request a MyHealthRecord.com account by sending an email to Registration.gateway@tachc.org or by asking a staff member to email you a registration link.
