What is Neighborhood Health?

Neighborhood Health offers primary medical care services including Family Medicine, Internal Medicine, Pediatrics, basic Gynecology as well as onsite laboratory services in most of our clinics. We provide regular physicals, well-child visits, urgent/sick visits, chronic disease management, women’s health, and preventive services such as vaccines and cancer screenings. We provide referrals for services that we do not offer including specialty care. We also offer medical care for patients with HIV. We do not offer obstetric services.

How to link my neighborcare account?

How to link accounts: Step 1 – Log into your Neighborcare Health MyChart account. Step 2 – Click Profile and then click Link My Accounts. Step 3 – After the page loads, scroll down until you see a Search box. Use the Search box where you can look up their other MyChart organizations.

What happens if you enroll in MyChart before your appointment?

If you enrolled in MyChart after your appointment, your provider will be added to your list of care team members after your next appointment.

When does MyChart remove proxy?

MyChart will automatically remove any adult proxy at midnight of the child’s 12th birthday. There are exceptions to the rule, such as adolescent patients that are developmentally impaired or differently-abled, or an emancipated teen.

What to do if you can't remember your password?

If you cannot remember your password, your password no longer works or you have too many unsuccessful log in attempts, you will need to reset your password. Please call your clinic to speak to a team member about getting your password reset.

At this time, you must contact Neighborcare’s Health Information Management department to access your full health care records. The team will safely generate your records and mail them to you.
