How do I connect to ProMedica care everywhere?

Connect with patient records from our facilities using Epic. ProMedica Link is a secure, web-based application that connects referring providers and their staff to their patients’ real-time electronic medical records. This includes lab results and imaging reports; notification of patients’ outpatient visits and hospital admissions and discharges.

How do I contact ProMedica medical records?

If you have an email address on file then your MyChart username will be sent to your email account. If you do not remember any of this information, or you do not have a valid email address on file, you will have to contact your MyChart help desk at (555) 555-5555 to help you regain access to your MyChart account. What is ProMedica MyChart?

You can make a secure payment online by using ProMedica MyChart. Choose one of the options below. Due to COVID-19, at this time, we are not releasing medical records in person at any of our locations. Other options for obtaining your medical records include via phone, fax, email and for MyChart users.

How to Access Your Medical Records

Completing a ProMedica Authorization Form will give us the permission we need to release your medical records to you. You can also use this form to release the records to another person or a doctor, if needed.
