Registration representatives are available Monday through Friday from 7 am – 5:30 pm. After hours, you may enter through the ER doors and check in at the nurse’s station.
MDH recognizes its responsibility to render necessary acute hospital care to all persons in need of such care, regardless of race, creed or economic circumstances. However, we also have a responsibility to the community of maintaining a financially viable institution. This responsibility can only be accomplished through the payment of all accounts in a timely manner. Our patient counselors are here to verify your insurance coverage and to assist the nurse case manager with your pre-authorization requirements. They can help you to understand your potential financial responsibility for hospital services.

Parents who want a caregiver to bring their children to MDH for treatment or who want children of driving age to present for tests on their own, must make prior arrangements with us to authorize consent for treatment and billing. In the event of a potentially life-threatening emergency, minors will receive treatment as directed by the physician.
At the hospital patient portal, you can access read-only medical information through our secure server. This includes educational materials, information on your current medications, allergies, immunizations, health issues, and lab test results. All information is pulled directly from your electronic health records.

Benjamin Goh, D.O. has over 25 years experience in Family Practice, wholly at Family Medical Centers’ Middleburg location. He is a retired colonel from he U.S. Army and is designated as an Aviation Medical Examiner by the Federal Aviation Administration.
Kent Braeutigam, D.O., received his Doctor of Osteopathic Medicine degree from the University of Osteopathic Medicine and Health Sciences in Des Moines, Iowa. He has more than 25 years experience in Family Practice.
Casey Padgett Todd, ARNP-C, received her Master of Science in Nursing from the University of North Florida, Jacksonville. She has served as a registered nurse providing one-on-one care to patients with chronic illnesses and to those needing emergency care. She has more than 15 years nursing experience.
Heather Bennett is an advanced registered nurse practitioner with Family Medical Centers serving at the Middleburg location. Bennett earned her Master of Science in Nursing from Chamberlain College of Nursing and her Bachelor of Science in Nursing from Slippery Rock University.
