FollowMyHealth® Patient Portal is a secure internet site which is protected through advanced encryption technology. Access to your account is secured through your own personalized user name and password, known only to you. It is important that you keep this information in a secure place and do not share it with others.
To delete your account, click “My Account” in the upper right-hand corner of your FollowMyHealth homepage. Then, click on “Preferences.” Then, click “Delete your UHR” under “Account Preferences.” The system will prompt you to confirm once more. If you so choose, click “Yes,” and your account will no longer be active.

Similar to calling the clinic, messages are routed to the nurse or medical assistants who work closely with your physician. The nurse or medical assistant may respond directly or route the message to the physician. All messages sent to the clinic become part of your medical record.
The patient portal should NEVER be used to report a life-threatening or urgent medical condition.
