What is the FCC patient portal and how do I access it?

How do I sign up for the patient portal?

Patient Portal access is now available. In order to sign up, visit our front desk and inform them that you would like to register for the Patient Portal. You will then be provided with a Patient Portal Agreement which will need to be signed and then you will be given an access code to register your account online.

What if a doctor doesn't have a patient portal?

Some medical groups and doctors do not use a patient portal and instead use different ways to stay connected with their patients. When that's the case, we work with them to make sure you have what you need to get in touch with them, anytime. You can find their information on your member ID card.

Fanno Creek Clinic is currently administering Moderna and Johnson & Johnson COVID-19 vaccines by appointment. Appointments are available for established patients and any of their friends and family members who are 18 years or older. We encourage everyone to be vaccinated to stop the pandemic.
