How do I make an appointment at FamilyCare specialists?

On your first visit, you will be asked to fill out registration forms. You may choose to pre-register by downloading, printing and completing the forms below. Just bring them to your first visit.

How can I communicate with my care team through the portal?

HOW CAN I COMMUNICATE WITH MY CARE TEAM THROUGH THE PATIENT PORTAL? Go to Messages on the left side of the screen, then click on Compose. Therapy services –Find your therapy provider in the drop down. These messages will go directly to the therapy provider.
