Crystal Clinic offers two patient portals to help you become a more active participant in your medical care. Each portal is a secure website that offers convenient 24-hour access to your personal health information. For our patients who have visited any of our clinics, we offer a widely used portal from Medfusion.
A patient portal is a website where you can review components of your Crystal Clinic Orthopaedic Center (CCOC) medical records. Using an internet web browser or a smart phone, you can review many records associated with your health care. This is a tool to help you better manage your health care.
You may also sign up directly for our Medfusion outpatient portal: Click the Patient Login button in the upper left. On the following screen, you will have an option to create your own portal account. Click the Create an Account button to begin your signup request. We then verify your patient relationship and approve your sign-up request.
At the patient portal sign-on screen, www.crystalclinic.com/portal, click one of the Forgot Password links in the blue box at the left of your screen. Follow the instructions on the forgotten password page of the portal.

With the use of the Representative function, you as the patient have the ability to designate access to some or all of your portal medical records based on YOUR assignment of the Representative’s access. You control what components are seen by the representative, and have the ability to change that access at your convenience.

Just as with the paper medical record, the legally appointed guardian of a patient may be designated as a representative for the patient’s medical records on their portal account if the scope of their guardianship permits access to medical information.
Some medical records, such as X-rays or the physician’s clinic visit notes, are not currently available in the patient portal. To access those records, you will need to make a request to your provider’s office.

Click the Sign Out link at the top right of the screen. Alternatively, if your keyboard remains idle for 10 minutes or more, you will receive a pop-up window asking if you are still actively using the portal. If you do not click the OK button, you will be signed out automatically. Any information you have typed and not saved or sent will be lost.
You can access the Doctors Care Patient Portal from any device with Internet capability to view your personal health information, communicate with your providers, view testing and lab results, or even print your work or school excuse.
Your visit information should be available within 12 hours of your visit. Testing and lab results will display as soon as 24 hours after your visit.
You will receive an answer within three business days. The Doctors Care Patient Portal should not be used for urgent situations.

Yes. Access can be granted to parents of children under age 18, as well as caregivers of elderly patients. Just contact the front desk staff at the practice. A unique email address is required for each person. Please note that proxy access to a child’s account will expire when he or she turns 18, and the child will need to create his or her own account.
