Discount Fee Program

The Community Health Center is pleased to offer financial assistance to eligible individuals and families through our Discount Fee Program. This program applies to medical, dental, and behavioral health services; however, our medical, dental, and behavioral health departments each utilize a different discount fee scale.

Becoming a Patient

If you are uninsured, please fill out the Patient Application and then call the Center to find out when you can meet with a Patient Services Representative to go over your paperwork. Once you have met with the representative, you can start making appointments. If you have insurance, please call the Center to schedule an appointment.

How long does it take for a pharmacy to process a refill request?

Please remember to allow 48-72 hours for the practice to process your refill request. You can also notify the practice of medications you are no longer taking or of any new medications. You can use the portal to notify the practice if you change your pharmacy.

Is Patient Portal compatible with mobile?

The Patient Portal website is compatible for use on various devices. In addition to a desktop site, a mobile site with a layout optimized for a smaller screen size is also available. The menus and screens are programmed to be easy to use on a smartphone and an icon can be placed on the Home screen for quick access.
