What can I do on the patient portal?

Coastal has a beautiful facility to deliver care, and even better doctors and nurses. Dr. Andrews has a calming and professional demeanor and is extremely knowledgeable on women's health. When I am at Coastal I know I am in good hands. All the women in my family see Coastal for their OB-GYN care and we have referred many friends here as well.

PATIENT FORMS

You will receive your patient paperwork via text and/or email once you have been scheduled for an appointment. Your first notification will be sent 5 days prior to your appointment. If you have not received your packet please contact our office for a new link.

RECORDS RELEASE

If you need your records sent to a facility outside of Coastal Women's Healthcare, please complete the release form below and fax it to us at 207-885-8499. We will complete your release as soon as possible.

New Patients

If you are a new patient and would like to establish care with Coastal Women's Healthcare, please call our office at 207-885-8400 to speak with a New Patient Representative. Once you have established a patient account with our practice, please fill out our medical records release form by following the link below.

Established Patients

All patient forms can be found on your secure patient portal account. If you do not have an account set up yet, please take a moment to do so prior to your next appointment. If you do not wish to create a patient portal account please plan to arrive at least 10 minutes early to your appointment to fill out necessary paperwork.
