Can I access the patient portal on my mobile device?

To enroll in mycareDOT™, you must first be an established patient of Clifton Park Family Medicine. You will need to provide our office with your name, email address, date of birth, and address. We will then send you an email inviting you to join the portal. Once you sign up, you can begin communicating with our office!

Can I still access my medical information from previous patient portals?

Any new information from visits to a St. Peter's Health Partners doctor’s office or hospital from May 8 and beyond, including prescription refills, visit notes and lab results, will now be in your new MyChart patient portal.
