How do I use the patient portal?

Welcome email has a link to patient portal login page of your practice. You can bookmark this link for future reference. In addition, you can also login to patient portal from ChARM PHR home page at https://charmphr.com. Note: ChARM PHR employs strict privacy rules. No one can look at the data except you or those designated by you.

How has your practice benefited from using charm EHR?

Patient Portal Registration You can register a patient portal account by accepting the invitation email sent by your practice. Note : If you have not received the invitation email, Check the SPAM folder. Contact your practice and check if your practice has enabled PHR account for you or not.

Patient Portal Login

Once the account registration completes, you will be automatically redirected to patient portal and also you will get a Welcome Email.

Patient Selection

After signing in you will land in Patient Selection page. This page shows the list of all the family members registered with your current logged in account. Select the patient whose information to be viewed.

Practice Selection

When you select the patient, the practices which the patient had already visited, are shown in the right side. Select the practice link to proceed.

Patient Portal Registration

You can register a patient portal account by accepting the invitation email sent by your practice.

Step 2

Choose your preferred username (Login ID) and password. Username and Password are case sensitive. Hence make sure that you enter them in proper case.

Patient Health Record (PHR) Portal

ChARM PHR is a Personal Health Record (PHR) portal to manage health information of patients, for themselves and their families. Patients can store their current and past medications, lab results, record health vitals and track progress using the built-in reports.

Integrated PHR and EHR

ChARM PHR is designed to be tightly integrated with ChARM EHR. This enables patients to share lab records with physicians and also allows patients to view the documents shared by their physicians. Patients can request appointments with their care team, which gets securely notified to the physician’s ChARM EHR account.
