Welcome to Our Practice

You are the reason we are here and do what we do. We are dedicated to making sure you feel cared for and important. In addition, we are dedicated to providing you with compassionate, high-quality medical care.

Medication Refills

Please contact your pharmacy for refill requests. Your pharmacy will fax us the request and your refills will be processed within 72 hours. You may also request refills on the patient portal.

Billing

We require payment of your co-payment and past-due account balances at the time of service. We accept cash, checks, debit and all major credit cards. (Visa, Master Card, American Express, Discover.)

Insurance

We will submit claims to most major insurance carriers. Please bring your insurance card and I.D. with you to every visit so we can ensure that our records are accurate.

Cancellations or Rescheduling

To better serve the needs of all our patients, we ask that you call at least 24 hours in advance to cancel or reschedule appointments. We make every effort to stay on schedule, although emergencies arise.

After-hours & emergencies

If you are experiencing a medical emergency, dial 911. However, if you need after-hours advice for a medical concern, please call (940) 591-6700 and our answering service will assist you.

Payment Information

We accept all major private insurances and medicare with a referral. We accept cash, credit, check, and online payments. A billing specialist is available to discuss any questions or concerns about payment information. Please contact us for more information.

Our staff looks forward to meeting you and discussing your healthcare needs. We strive to keep our appointments on time, as scheduled. Prior to your appointment, we encourage you to fill out the patient forms that are located on this website so that you can accelerate your check-in process.

Gynecology Services

We are focused on your overall health at your well-woman exam that may include a pelvic exam, pap smear, breast exam, and a referral for a mammogram.

Obstetrics Services

Congratulations on your pregnancy! We are pleased that you have chosen CCW for prenatal care, during your pregnancy, and for the birth of your baby.

Urogynecology Services

We have state-of-the-art diagnostic tools and treatment techniques for patients with a pelvic floor disorder (cystocele, rectocele, and perineocele).

Surgical Services
