Calvin’s Health Services is able to manage the medication treatment for students with ADHD. To expedite the transfer of care to one of our physicians, it is important to have all of the student’s medical records transferred to our office prior to that first appointment.

Who is the team physician at Calvin University?

Dr. Edwin Kornoelje, Calvin’s team physician, is available most Thursday afternoons for evaluation of musculoskeletal injuries and rehabilitation for Calvin University students and athletes. He is also available for appointments at his private office.

How do I access information from the previous patient portal?

Previous Portal Users will need to create an account with MyHealthHome in order to continue accessing their personal health information. Information contained in the previous patient portal can still be accessed by contacting your local hospital. Click here for information on how to do that.

My Hospital Portal

Records of your inpatient hospital and/or surgery summary, labs and radiology results, discharge instructions and pay your bill on-line.

When you call your providers office, select option #1 from the menu to reach the front desk.

Two Ways to Create Your MyHealthHome Account

Enrollment Invitation – Flowers Hospital will send an email to the email address you provided upon registration to the hospital. Follow the instructions in that message to create your account.

Connect to your MyHealthHome Account
