A proxy is a parent or legal guardian of a child; the legal guardian or holder of a health care power of attorney for individuals age 18 or older; or another legally recognized Personal Representative. A Personal Representative is a person who has the authority under state law to consent to treatment on behalf of the patient; including court-appointed guardians and parents on behalf of unemancipated minors.

On the day you visit our Denver area dermatology and Mohs surgery practice for your appointment please bring your insurance card. Be sure that any required referral has been processed. Insurance co-pays are expected to be paid at the time of your visit.
