How do I request my medical record?

If you are seeking a copy of your medical record, a request must be made in writing to the Bertrand Chaffee Hospital Health Information Department. The request should indicate that a qualified person is making the request and should be as precise as possible.

Can my records be sent to a third party?

Yes, we would need a similar request in writing with the name, address, phone number and fax number of the physician or third party that you would like your record to be sent to. The request must be signed.
