How old do you have to be to communicate with FPC?

Parents/guardians may communicate with FPC providers and staff as you always have until your child reaches age 18.

Can a parent/guardian be a portal authorized representative?

For this reason, we find it necessary to ask our patients in this age group to complete a consent form naming a parent/guardian as their portal authorized representative. Without this consent from the minor patient, we will not be able to give parents/guardians access to the minor's portal. (Click the link below to access the form.)

For example: Wives, please do not message us from your patient chart about your husband's healthcare. This is a violation of privacy and the message will be immediately stricken from your chart.
