Enter Employee ID located on your paycheck or paystub under File (see screen shot below). Do not use the leading zeros. It should be a 6 digit number (blocked out on screen shot for security). Another source to find your Employee ID is on the Meritain card, listed as Member ID.

APD iConnect is being implemented in phases. The first rollout (Winter 2018) included APD staff and Waiver Support Coordinators (WSCs). In 2020, we progressively included other providers. WSCs & APD staff began using APD iConnect for budgets, cost plans, authorizations, and SANs.
If you would like to sign up and don’t have an After Visit summary from your physician or a Baptist Health facility, visit MyChart to get an activation code. Once you have your activation code, you can setup your username and password on the MyChart website .
To login to MyChart, visit mychart.baptisthealth.com. Once on the page, enter your username and password that you setup when your created your account and click Sign In.
