Our Patient Portal is powered by FollowMyHealth and it is a secure, convenient way to access and manage personal health information and communicate with us online. The portal can be used to manage your health information and the information of family members as well. It’s a free tool that allows you to:

How to add minors to my certified allergy?

You can add the health information of minor children to your portal account. Simply call our portal support line at 518-429-2650 or email portalsupport@certifiedallergy.com to request access as a proxy.

How to delete a portal account?

The option to delete the account can be found under the My Account tab, within the Preferences section. Simply select “Delete the UHR”.

Does FollowMyHealth use encryption?

Yes. FollowMyHealth uses encryption techniques to protect your information while in transit and while stored in your account. This information is only accessible using the password you created.

Can you delete medical history on portal?

You have complete control over the information contained in the portal account. You can add and delete conditions, medical history, medications, vital signs and allergies. Any change you make to these items will not be transmitted to our office. You can send this information to us in the form of a message using the Send Message link at the top of the page.

Can an adult patient access patient information without consent?

In cases where an adult patient may be mentally impaired or in need of supervision, access to patient information may be granted to a responsible adult without consent.
