You can complete the form through our patient portal. If you need assistance, please let us know and we will assist you in the office with a tablet.

Cancellation Policy

We will set up a specific course of care for you. A certain number of appointments will be determined and are necessary for us to get the results we both desire. Thus, keeping your appointments is imperative! If you cannot make your scheduled appointment please call and let us know and we will get you rescheduled.

Financial Policy

We are not enrolled with any medical health insurance company contracts. Due to the wellness and preventative nature of our care, our model of care does not fit the insurance model. We will be happy to provide superbills for insurance reimbursement.
