To login to MyChart, visit mychart.baptisthealth.com. Once on the page, enter your username and password that you setup when your created your account and click Sign In.

The MyChart portal offers patients the ability to securely view and print/download portions of their medical records , such as physician notes and diagnostic testing results. The content currently includes most clinical notes and test results. The use of the MyChart portal is FREE. Your information is securely stored on MyChart indefinitely.

All requests for the release of medical records must be submitted in writing and must be dated and signed by the patient or the patient's legally authorized representative. In the case of a minor, the parent or guardian must sign the authorization.
