To view your medical records in Patient Fusion, your doctor must use Practice Fusion as their electronic health record (EHR) system and must grant you access to your records. 1. When your doctor's office grants you access, a registration email will be sent to the email address on file with your doctor's office.
Practice Fusion has a built-in patient portal - Patient Fusion - which allows patients to electronically access their medical records. Available information includes the patient's medication list, diagnostic history, allergies, immunizations, and appointments (past and upcoming). In addition, you can elect to share some structured lab results.
As soon as your doctor enrolls you in Patient Fusion, you should receive a registration email containing a link that you'll need to create your account. If you did not receive an email, check your spam or junk email folders.

In the event that the patient loses their PIN, you may locate it within their chart by clicking on the Patient Portal enrollment status at the top of their chart. The PIN will be listed under the Access code column (see Graphic 5 below).
