What is ‘You and PAH’ Patient Portal? The You and PAH’ patient portal is an electronic collection of your healthcare related information. The ‘You and PAH’ portal allows you to ‘look into’ your electronic medical record and see information from any of your Punxsutawney Area Hospital interactions.
To enroll we will need your email address on file at the hospital and you will need to get your medical record number. To obtain your medical record number and provide your email address, please go to the Medical Record Department at PAH. You may also do this during any registration for services at the hospital at your time of treatment.
PAH provides free aids and services to people with disabilities to communicate effectively with us, such as: qualified sign language interpreters and written information in other formats (large print, audio, accessible electronic formats, other formats).

Currently, anyone over the age of 18 is permitted to create a patient portal account. Among this group, anyone legally authorized to view another person’s health information may be given access to the patient portal for that person.
