Patient Portal & Forms Our Patient Portal was created specifically for you as a Pacific Rim Orthopedic Surgeons patient and gives you the opportunity to complete as many forms as possible online, to save you time in the office during the registration and check-in process.

Patient Forms

Most of our forms and patient intake is now completed via Phreesia, our online check-in platform. If you are unable to complete your registration and check-in online before your appointment, we will happily provide you with the forms to complete in the office on the day of your appointment.
