This can be found on hospital billing documents (preferred) or acquired through an Authorization for Release of Medical Information form through our Health Information Management department.

Log in to the portal using your ONID username and password. New student data is loaded into the patient portal two terms (6 months) prior to your scheduled start date. If you attempt to login too far in advance, the system will not allow it.
