Our accessible and user-friendly online patient forms are designed to support your care and ease your experience.

Pay your bill

You can easily pay your bill online. We offer online bill pay for both OrthoNY and the Executive Woods Ambulatory Surgery Center. Before making your selection, please check your billing statement for the correct service provider.

Health insurance

If we participate with your insurance plan, we will submit your claim. If we do not, as a courtesy, we will submit the claim on your behalf and you will continue to get a statement from us until payment is made.

Managed-care plans

We participate with many managed care plans. The key to smooth processing of these claims is the Referral Form from your Primary Care Physician. Under these plans, to see us for an office visit, you must provide a Referral Form or sign a Statement of Personal Responsibility. Co-pays, if required, are expected at the time of service.

Medicare

We welcome Medicare patients and are pleased to submit your Medicare and any secondary insurance claims.
