If you are a new patient to ONE Health Ohio or if you have not visited us in the past 12 months, please print and complete the appropriate patient packet and bring with you when you come for your visit. This will save you time in having to complete the paperwork during your appointment.

The health center services all ages, including adults, teens, children and infants. Falls Family Care is located at 175 E. Broad St., Newton Falls, OH 44444. To schedule an appointment, please contact us at 330-872-2010. Hours of operation: Mon, Tues, Wed & Fri 8:00am-5:00pm and Thurs 9:00am-6:00pm.

Our American Academy of Pediatrics Board Certified physician and other providers follow these guidelines to ensure better health outcomes for your child.

Proxy

If you are the parent or permanent legal guardian of a patient that is a child between the ages of 0-11 years old or a legal guardian of an adult patient or a person who has a Power of Attorney for healthcare over an adult patient, you can request proxy access.

To self-register for a FollowMyHealth portal account, go to https://colnhc.followmyhealth.com, click “Create an Account”, choose your login method, fill out account information, click ‘Connect’ to PrimaryOne Health, then ‘Next’ to accept the terms and conditions.
