Medical records can be transferred to Ocala Family Medical Center by sending your current provider a completed authorization. Use the Medical Records Release form found in the Patient Forms section above.

If you do not have a scheduled appointment and wish to sign up for My FMC Portal, you need to go to the FMC Medical Records Department, which is located on the first floor at 401 N. Ewing St. in Lancaster, Ohio. The hours are 7 a.m.-8:30 p.m.
