Fast Healthcare Interoperability Resources

FHIR is Fast Healthcare Interoperability Resources. It’s a standard that enables applications to retrieve clinical data from Electronic Medical Record systems, such as Epic, using APIs. An API is an Application Programming Interface which gives 3 rd party applications access to data within Epic.

Why are we seeing patients in office?

We are seeing most patients in office as we feel that this provides the best and highest level of care.

How to get previous medical records?

In order to get previous medical records, we need a signed release form. You can find our Medical release form under our Patient Forms tab to send to the previous practice in order to obtain records.

What information do you need to get a referral?

In order to obtain referrals, we must have your patient registration paperwork (specifically insurance information), along with the name of the physician you are seeing, the location of the clinic and the reason for the visit.

Do you need to fill out a new patient registration form?

ALL patients must fill out new patient registration paperwork prior to their first visit. This is a new practice and for that reason, we need to have these forms on file. Much of this information is necessary in order for us to refill medications, obtain referrals, order lab work and bill insurance.
