Oak Valley Hospital is a full service, non-profit public hospital created to provide residents of Oakdale and the surrounding rural communities with access to superior quality medical information, treatment, and care.

Is Oakdale Nursing and Rehabilitation Center open?

The Oakdale Nursing and Rehabilitation Center is open for family visitation seven days a week by appointment at designated times. To schedule an appointment at the main facility please call 848-5303 Monday through Friday during normal business hours. For the Transitional unit please call 848-5308.

How to make a request

Please download the form below to obtain a copy of your medical record. You may also use this form to have your medical records sent to another medical facility.

Processing a request

Records are usually available within 7 to 10 business days from the time the request is received. Please note this at the top of the request form if you are interested in picking up your medical records in person. A photo ID will be required to pick up a request that you have submitted.
