Who must sign non behavioral health information?

For non-behavioral health health information requests both a parent or legally appointed guardian and minor must sign. For behavioral health requests including Patient Access the minor must sign. If the minor can consent to treatment, the minor can sign the request.

How old do you have to be to request medical information?

If the patient is a minor under twelve (12), the parent or legal guardian may request (sign for) the health information. If the patient is a minor twelve (12) years of age and under eighteen (18), the request must be made (signed) by: The parents or legally appointed guardian.
