Pre-Appointment Phone Call

In order to shorten the length of your stay in our clinic during your appointment, we will make three attempts to call you within a week of your appointment to obtain the needed information and complete the new patient forms with you over the phone if you are a new patient.

Follow-Up For Surgical Visits

If you require a follow-up visit or surgery following your initial visit, our staff will help you to obtain the necessary information and pre-authorizations or pre-determinations form your insurance company. Insurance pre-authorizations or pre-determinations do not guarantee payment.
