To allow a family member access to your health information on MyHealthConnection Patient Portal, you will need to fill out the appropriate MyHealthConnection Proxy Access form and have the family member sign it. Bring it to Novant Health UVA Health System Culpeper Hospital's Medical Records department. Once this form is filled out, it will be scanned into your electronic record to show that you granted permission to a proxy. If the family member does not currently have a MyHealthConnection account, then he or she will need to provide an email address to receive portal notifications.
