Once we verify that you have met all the immunization requirements, you will receive a secure message through the patient portal indicat ing that you are compliant. If you are missing any required vaccines, the secure message will indicate that you are non-compliant and will include instructions.
In addition, students coming from countries of high tuberculosis (TB) incidence will be tested for TB at UHS. (If you have results of an interferon-gamma release assay TB blood test, such as Quantiferon Gold, performed in a lab in the US, within 6 months prior to arriving on campus, you can submit; otherwise, you will be tested at UHS.)
A physical exam is not required. We do ask that you complete the online health questionnaire located inside your patient portal, which helps our providers take care of you when you come to UHS for medical care in the future.
One of the features of MyChart is Proxy Access. Obtaining access to another persons’ UHS MyChart account that is NOT their own is called Proxy Access.

Please note, your UHS medical information for approximately the past 3 years only will be in your MyChart account. If you have questions about your prior medical history, please talk with your provider.
If you are a student and have received a first dose of the Moderna or Pfizer vaccine, upload a picture of your vaccine card to your UHS patient portal. (Use the "My Forms" feature and click "COVID Vaccination - Students.")
After a second dose of Moderna or Pfizer, or a single dose of the Johnson & Johnson, upload a picture of the vaccine card through the "My Forms" section of your UHS patient portal.
Documentation for students who received the booster at the on-campus clinic was automatically uploaded to the student’s UHS patient portal.
