What is the Northwest Health Patient Portal?

Northwest Health provides a Patient Portal help you be informed about your care and what you need to do to stay healthy. The Patient Portal is a secure, online home for your inpatient hospital health information.

How do I log in to my Patient Portal account?

To log in to your Patient Portal account, please select the hospital where you received care. Click here for frequently asked questions (PDF). If you do not find the information you need in the pdf document for which a link is provided above, please call the toll-free support line at 1-855-624-2844, Option 2.

Does northwest health services accept Medicare or Medicaid?

In Healthcare. Northwest Health Services is a not-for-profit FQHC (Federally Qualified Health Center) serving 15 counties in Northwest Missouri. We accept Medicaid/Medicare, Private Insurance/Pay, and offer a Sliding Fee Scale for individuals and families who are uninsured.

Two Ways to Create Your MyHealthHome Account

Enrollment Invitation – Northwest Health will send a text and / or email to the email address and/or cell phone number you provided upon registration to the hospital. Follow the instructions in that message to create your account.

Note to previous Portal Users

Previous Portal Users will need to create an account with MyHealthHome in order to continue accessing their personal health information. Information contained in the previous patient portal can still be accessed by contacting your local hospital. Click here for information on how to do that.
