How do I create an account with Northwest HealthCare?

Enrollment Invitation – Northwest Healthcare will send an email to the email address you provided upon registration to the hospital. Follow the instructions in that message to create your account. Self-Enrollment – If you did not provide Northwest Healthcare with your e-mail address, you can still create an account by clicking here.

What is included in the NMC patient portal?

NMC’s Portal covers all NMC hospital services as well as NMC’s physician and specialty practices. NMC’s Patient Portal Consent Form in PDF Format is Available Here. In addition to the NMC MHealth Portal Application, NMC is pleased to provide you access to your health information through other Personal Health Record Applications on your smart phone.

How to access my health home?

My Health Home patient portal is a secure, online home for all your health information at Northwest Health - Starke, whether it be from an inpatient stay, outpatient visit or physician appointment. In the My Health Home, you can: 1 View and share your personal health information related to your hospital, outpatient or office visits. 2 Have electronic access to your list of medications, diagnoses, allergies, lab test results, radiology results, patient history, and other care information related to your visits. 3 Access your hospital discharge instructions for better after-discharge care. 4 Manage family members’ health information, if authorized.

Faster Results Through Your Patient Portal

Faster Results: Tests and reports are now available as soon as they are finalized. Prior to this, some laboratory results and imaging reports were not available immediately after they were finalized. This change means that you may see test results before your care team is able to review them.
