We are pleased to offer you a secure, interactive patient portal that lets you communicate with us from your home or mobile phone. With our patient portal, you can send us a message, review your lab or x-ray results, request an appointment, pay your bill online and many other services.
We are excited to now offer a limited number of telemedicine visits for established patients. This service will allow us to offer a convenient manner for virtual face to face visits. Please check with your insurance provider to make sure your policy covers these types of services. These visits must be scheduled in advance.

Shore Patient Portal is a secure website where you can conveniently access your health information related to your visits at Shore Medical Center.
Do not use My Shore Portal in the event of an emergency, For Medical Emergencies, please call 9-1-1 or your physician’s office immediately.
If you need urgent medical assistance, call 911. If you need a response from the office within 24 hours, please call the office. It may take two to three business days to receive a response to any FollowMyHealth communication with the office.
If you do not specify a timeframe, the office may schedule the appointment based on physician availability. Appointments cannot be scheduled, cancelled or rescheduled on the same day.
You may see medications that have been prescribed by physicians other than a Family Medical Center physician, such as a specialist. Prescription refills can only be requested on medications that have been prescribed by your physician or one of their partners.
Step 1: To connect your MyChart with another participating facility, simply hover over the “Health” icon after you have logged in and navigate to the menu option “Share My Record”. (See Step 1 & 2 below) Step 2: You will then be directed to the page seen above in Step 2.
Sign Up For MyChart: In order to sign up for MyChart, you will need to get an activation code; use one of the options below to get access: Fill out the form online and receive your activation code via email. (if there are issues, please call us directly (208) 954-8730.)
