We believe that every patient deserves to be informed and prepared before their diagnostic imaging examination. We provide insurance information, and all required forms so your visit to North Star is as quick and easy as possible.
NSI does its best to accommodate all patient diagnostic imaging requests in a timely manner. We recognize that many of our patients work 9–5 jobs and that it can be difficult to make an appointment during the day.
We are glad to have you as a patient and hope you will enjoy being able to access your imaging reports and schedule an exam quickly and easily online. Currently, only screening mammograms and general x-ray can be scheduled through the portal.

Please print the forms below that are specific to your diagnostic imaging procedure. Complete them and bring them with you to your appointment. You must also present a form of ID (Driver’s License, State-Issued ID, etc.), Insurance cards, and a method of payment for any applicable co-pay or deductible.
NSI accepts most insurances. We have provided a list of the major insurance plans we accept and links to their website. If you don’t see your insurance plan listed below, please contact us to verify coverage.
