FINANCIAL & PAYMENT POLICY

If you do not have insurance, payments for medical care are to be made at the time of the visit. If a large bill is anticipated, a payment program may be arranged ahead of time with the first partial payment made on the day of service.

INSURANCE BILLING

Patients are responsible for all charges incurred at the time of visit. Those patients wishing to use insurance will need to provide all necessary insurance information prior to the scheduled appointment and will be responsible for a co-pay or deductible as applicable.
